Resident Safety Review Council
On March 1, 2012, the Oregon Legislature passed House Bill 4084, which called on the Oregon Patient Safety Commission to chair a short-term work group called the Resident Safety Review Council (RSRC). The Commission partnered with members of the work group to fully understand the current system, review the methods used to investigate abuse reports within the long-term care community, and evaluate the current statutory definitions for abuse. The work group’s goal was to provide recommendations for improvement and maximize protection for vulnerable adults.

Root Cause Analysis Materials for Long Term Care Facilities
The Commission, in partnership with Oregon’s Patient Safety Improvement Corps, developed Oregon’s Guide to Root Cause Analysis in Long Term Care. The guide is intended to help long term care facilities conduct event investigations based on root cause analysis methods and current patient safety concepts.
Long Term Care Falls Investigation Toolkit
In response to the number of adverse event reports related to falls, the Patient Safety Commission and community partners developed a simple, easy-to-use guide to investigate and reduce recurrence of such falls. The guide is structured around Root Cause Analysis (RCA) and incorporates other evidence-based quality improvement principles.
Transitional Care and Pressure Ulcers Project Toolkit
The Oregon IHI Network, including the Patient Safety Commission, worked with Oregon leadership of Advancing Excellence in Americas Nursing Homes to develop cross-setting interventions for transitional care, with an initial focus on pressure ulcers. We support use of this toolkit to improve pressure ulcer prevention and care in Oregon.
