Submit a Report
Fill out the Adverse Event Reporting Form for Oregon hospitals (file may take a few seconds to load). Choose the correct harm level to display all required fields. Choose the correct harm level to display all required fields. Once the report is completed, print the report and submit it to the Commission via fax to 503-224-9150.
Note: All reporting forms must be submitted via fax as our online submission system is temporarily closed due to a system update. Please submit all reports to FAX: 503-224-9150.
