REPORTING AN EVENT

The Oregon Patient Safety Commission is currently enhancing the adverse event reporting system, a core component of Oregon’s Patient Safety Reporting Program. Changes to the reporting system will enable participants to more easily report the events that may cause patients harm. System enhancements will also improve the Commission’s ability to analyze reports and provide feedback that can help participants learn from adverse events and improve patient safety in Oregon. While the system is undergoing enhancements, participants can still submit adverse event reports. For instructions on how to submit reports, please visit the SUBMIT REPORTS page.

RECOGNITION TARGETS FOR 2012

The Commission’s recognition targets are intended to guide healthcare organizations participating in the Patient Safety Reporting Program. Targets are designed to change each year as organizations build their reporting programs to meet the State of Oregon’s reporting requirements. Targets are also designed to ensure that the Commission receives enough adverse event reports to build a strong database for learning and recognize healthcare organizations for their transparency efforts and commitment to patient safety.  Each year, the Commission identifies leading participants in each reporting category and issues awards to the top performers based on established recognition targets.
