Having an open conversation after experiencing harm in a healthcare setting can help you get answers about what happened and why, as well as learn about steps being taken to improve care for future patients. EDR conversations are confidential under Oregon law, allowing all parties involved to feel more comfortable discussing what happened.

EDR may be the right choice if all of these apply:
You or a loved one received medical care that resulted in serious physical injury or death.
The event occurred in the state of Oregon after July 1, 2014.
You are the patient (or the patient's representative).
The event occurred in a healthcare facility or involved a healthcare professional.
You want to talk to your healthcare provider about what happened.
How does it work?

Step 1: You request a conversation.
EDR starts with a Request for Conversation, which you (or your representative) fills out and submits online.

Step 2: We contact the healthcare provider(s).
We will notify the provider(s) included in your request until we receive a response. EDR is voluntary for everyone involved, and a request to have a conversation may not be accepted.

Step 3: The provider(s) make the next move.
If your request is accepted, the provider will reach out to you to schedule a conversation. We will not participate in any conversations, but we are always here to provide information about how the process works.

While we're not involved in the conversations, we'll stay in touch to learn about your experience. Your feedback will help us improve this process for others.

We are the Oregon Patient Safety Commission (OPSC), and we exist to help build safer care for patients.
As a semi-independent agency of the state of Oregon, we offer patient safety resources and support to ALL Oregonians. Our Early Discussion and Resolution (EDR) process helps any Oregonian who has experienced harm within the healthcare system by connecting them with their healthcare provider so they can speak candidly about what occurred, work toward reconciliation, and contribute to preventing harm from happening to others.
