OPSC is a non-regulatory Oregon state agency that provides patient safety resources, learnings, and support to all Oregonians. Our job is to help folks who have experienced harm within the healthcare system. We serve both the patient and healthcare community by providing programs and resources to help everyone safely and confidentially talk through harmful medical events and work toward resolution. helping make sure others don’t experience similar harm in the future.

Help for Patients & Families

Have you or a loved one experienced harm in a healthcare setting?

Our EDR program helps connect patients who experience harm and their healthcare provider so they can speak candidly about what occurred, work toward reconciliation, and contribute to safeguarding others from similar harm.

Help for Healthcare Professionals

We believe healthcare facilities throughout Oregon should be able to learn from each other’s experiences as catalysts for improving overall care. Our Patient Safety Reporting Program (PSRP) collects and analyzes information from facilities about patient harm events — and we share the lessons learned to support facilities in improving care and preventing future harm.
