Help for patients & their families through open conversation

If you experience serious harm as a patient in Oregon, you or a family member can request a conversation with the provider through Early Discussion and Resolution (EDR).

Have your voice heard outside of the legal system

Here's how it works:

We connect you with the healthcare provider so you can ask questions about the harm that occurred and share how it affected you.

Talking candidly can help you and the provider restore trust and work toward reconciliation.

Because communication through EDR is protected under Oregon law, you may be more comfortable talking about these events.

The information you share may contribute to strategies for preventing future harm and improving patient care.

Is EDR right for you?

EDR may be right for you if all of these apply:

  • You want to talk to your healthcare provider about what happened.

  • You are the patient or the patient’s representative.

  • The event happened in Oregon after July 1, 2014.

  • The event happened at a healthcare facility or involved a healthcare provider.

  • The care resulted in serious physical injury or death.

What if EDR isn’t right for you?

That’s ok. We have other resources to help:

Request the conversation and we’ll make the connection.

You can request a conversation with a provider through EDR . You can submit this request online, by mail, or by phone. The provider can also initiate an EDR conversation.

If you both agree to talk, the provider will work with you to arrange the time and location. OPSC does not participate in EDR conversations, but we can provide information about how the process works.
