About EDR
Despite best intentions by healthcare facilities and providers, serious medical harm sometimes occurs. And when this happens, patients (or their representatives) in Oregon can request a conversation with the provider or facility involved through Early Discussion and Resolution (EDR). This gives them the opportunity to speak candidly about the harm that occurred and work toward reconciliation outside the legal system.
EDR is voluntary and confidential, and it gives patients the power to ask important questions about the care they received.
EDR is available to all Oregonians: We want them to know about it.
EDR is available to all Oregonians. But… not all Oregonians know about EDR. To bridge that gap, we are asking you to help us spread the word. We want to ensure that our valued healthcare and community partners have all the tools they need to answer questions about EDR. Please explore the resources on this page, and let us know if there are any additional tools or documents you need so that you can share the availability of EDR with your communities.

Who is OPSC?
We are the Oregon Patient Safety Commission (OPSC), and we exist to help build safer care for patients.
As a semi-independent agency of the state of Oregon, we offer patient safety resources and support to ALL Oregonians. Our Early Discussion and Resolution (EDR) process helps any Oregonian who has experienced harm within the healthcare system by connecting them with their healthcare provider so they can speak candidly about what occurred, work toward reconciliation, and contribute to preventing harm from happening to others.
