OPSC Honors Patient Safety Leaders in Oregon

Honorees Recognized at the Seventh Annual Oregon Patient Safety Forum

Portland, OR—On March 16, 2018, the Oregon Patient Safety Commission (OPSC) honored healthcare organizations at the Seventh Annual Oregon Patient Safety Forum for their strong patient safety work in 2017. Honorees included healthcare organizations that have made an outstanding commitment to transparent communication with patients and families, organizations that have actively engaged with Oregon’s Patient Safety Reporting Program (PSRP), and, for the first time, organizations that have dedicated time and resources to addressing healthcare-associated infections (HAIs) through their ongoing engagement in OPSC’s Quality Improvement (QI) Initiative work. 

Leading Through Transparency Honorees

This year, OPSC was delighted to present the second annual Leading Through Transparency awards, honoring Oregon healthcare organizations that made an outstanding commitment to transparent communication with patients and families during 2017. This commitment is the cornerstone of OPSC’s Early Discussion and Resolution program. The 2017 Leading Through Transparency honorees were:

  • Grande Ronde Hospital

  • Providence St. Vincent Medical Center

PSRP Honorees

PSRP is a program designed to analyze and understand why and how patients may have been harmed during medical care and to share successful strategies for making care safer. This year’s honorees each exceeded 2017 recognition targets established by OPSC and stood out among their peers.

PSRP Leading Participants

Seven healthcare organizations were named OPSC’s 2017 Leading Participants for consistently analyzing patient harm events and for their contributions that make shared learning possible:

Ambulatory Surgery Centers

  • Interstate Ambulatory Surgical Center (Kaiser Permanente)

  • Skyline Ambulatory Surgery Center (Kaiser Permanente)

  • Sunnybrook Ambulatory Surgery Center (Kaiser Permanente)

Hospitals

  • PeaceHealth Sacred Heart Medical Center at RiverBend

  • Providence Milwaukie Hospital

  • Providence Willamette Falls Medical Center

Nursing Facility

  • Providence Center for Medically Fragile Children

PSRP Honorable Mentions

Two healthcare organizations received honorable mentions for their contributions to PSRP:

  • Salem Health

  • Wallowa Memorial Hospital

Other healthcare organizations that exceeded or met the 2017 PSRP recognition targets were acknowledged at the event.

QI Initiative Honorable Mentions

Ten long-term care facilities were also recognized for demonstrating their commitment to patient safety by dedicating time and resources to addressing healthcare-associated infections (HAIs). These Honorees are participants in OPSC’s Healthcare-Associated Infection Prevention Collaborative work, which is supported by the Oregon Health Authority via funding from the Centers for Disease Control and Prevention.
The willingness of these organizations to collaborate with one another is essential in effecting systematic change throughout the State. The 2017 QI Initiative Honorable Mentions included:
