Reports on Oregon Patient Safety Programs Released by OPSC for 2017

Portland, OR — The Oregon Patient Safety Commission (OPSC) released the 2017 reports for two of its core programs—Early Discussion and Resolution (EDR) and the Patient Safety Reporting Program (PSRP). OPSC is committed to sharing information from both EDR and PSRP to promote shared learning to help prevent patient harm and encourage a culture of patient safety across Oregon.

Early Discussion and Resolution

In its first three years, EDR received 100 Requests for Conversation from both patients and healthcare professionals. When things go wrong during healthcare and serious injury or death occur, an open conversation about what happened can move both patients and healthcare professionals towards resolution. Oregon’s EDR program offers support and legal protections for these important conversations. By encouraging transparency and accountability in healthcare, EDR may also promote system improvements that benefit future patients.

The new EDR report provides an overview of three years of EDR activity and offers lessons learned during implementation, as well as recommendations for improvement.

Patient Safety Reporting Program

In 2016, Oregon healthcare organizations—ambulatory surgery centers, hospitals, nursing facilities, and community pharmacies—voluntarily contributed 564 adverse event reports to PSRP. Because these events are prime opportunities to learn and improve, PSRP is designed to identify the core reasons why events are occurring and strategies to reduce recurrence of similar events. OPSC then shares de-identified PSRP information across the state so that broader learning can occur.

The annual PSRP report provides information about the most frequently reported events in 2016, the factors that contributed to those events, and OPSC’s recommended focus areas for healthcare organizations that are dedicated to making care safer for their patients.
