Help for Oregon Healthcare Providers

OPSC provides two programs to help healthcare providers and their patients with their experiences—and creates a safe space for reporting to reduce the number of incidents moving forward.

Despite everyone’s best intentions, patient harm sometimes occurs. Our programs offer a constructive space for healthcare facilities, providers, and patients to confidentially discuss the events, learn from what happened—and make care safer for everyone going forward.

EDR may be able to assist

If someone experiences serious harm or dies while under your care, Early Discussion and Resolution (EDR) connects you with the patient or their family to talk confidentially about the harm that occurred.

This conversation is a chance to acknowledge their experience and share how you plan to prevent similar harm going forward.

Your shared experience can make a difference

Through our Patient Safety Reporting Program (PSRP), healthcare facilities throughout Oregon can learn from each other’s experiences by voluntarily providing information about serious patient harm— or near misses— as well as their strategies for preventing future events. We then analyze these details to better understand how and why the harm occurred and share the broader lessons learned. We believe healthcare facilities throughout Oregon should be able to learn from each other’s experiences with patient harm so we can work together to improve overall care.
