The PACT Collaborative is a 12-month learning collaborative dedicated to helping organizations establish a highly reliable, holistic process for responding to and learning from patient harm.

Opportunity to Join PACT’s First Oregon Cohort
The PACT Collaborative Oregon Cohort offers an exciting opportunity for Oregon healthcare organizations to support and learn from each other within the context of Oregon's healthcare system. This cohort will become a foundation for continued collaboration and encourage the equitable spread of Communication and Resolution Programs throughout the state to improve how organizations respond to and learn from patient harm.
