UTI Prevention Resources for Long-Term Care Facilities

These tools and resources were implemented by the Oregon Stop UTI Initiatives, AHRQ Long-Term Care Collaborative and Portland Metro Collaborative Pilot, and are now intended to provide guidance on the development and application of UTI prevention projects in long-term care facilities.

Data Collection Tools for National Healthcare Safety Network (NHSN) Users

National Healthcare Safety Network (NHSN)
CDC’s NHSN website.

Modified NHSN UTI Reporting Algorithm Information Sheet
An algorithm that helps determine which UTI events are reportable.

Modified Denominator Collection form (Excel)
Forms modified to capture the required denominator for data that was reported during the Stop UTI Initiatives which can now be used as a model for other projects.

UTI Surveillance Criteria Checklist
A checklist of possible symptoms that may be present at the time of UTI diagnosis to help track and diagnose an infection based on a resident’s symptoms.

Antibiotic Start Spreadsheet (Excel)
A form to track prescriptions and record adherence to minimum criteria for initiation of antibiotics in residents.

Facility to Provider Communication Form
A faxable form that can be used by a physician prescribing antibiotics to document the signs/symptoms and lab results that were used to determine treatment - can also be used internally for tracking reporting criteria.

A checklist to ensure proper care each time a new indwelling urinary catheter is inserted.

A mnemonic for steps to reduce catheter-associated urinary tract infections.

A brochure to assist facility leadership in engaging residents and families in their care.
